Full Time Registered Nurse opportunity open for Home Health visits throughout New Castle County.

Working hours:

Monday - Friday 8:30 – 5:00 PM.

• Follow a designated care plan according to patient needs.
• Make home visits to clients in designated geographic territories.
• Perform assigned duties, including administration of medication, wound care, treatments, and procedures.
• Monitor clients' conditions and report changes to the Clinical or Client Services Manager.
• Execute doctors' orders and properly document them.
• Perform client assessments as necessary.
• Case management and coordination.
• Accurately document observations, interventions, and evaluations using a touch pad tablet.

• Current license as a Registered Nurse in Delaware.
• Minimum one year of recent, verifiable clinical (medical/surgical) experience.
• Prior home care experience strongly preferred.
• Graduation from an accredited nursing program.
• Ability to work independently and manage time effectively.
• Strong interpersonal skills.
• Solid computer skills; EMR experience preferred.
• Ability to travel to cases as assigned.
