Polaris Healthcare and Rehabilitation Center, a 100-bed facility located at the Milford Wellness Village, seeks a compassionate and experienced RN Unit Manager to manage the unit, utilize the nursing process to assess, plan, implement, evaluate resident care, and ensure high quality, compassionate care.

• Utilize the nursing process to assess, plan, implement, and evaluate resident care.
• Assess signs and symptoms indicating changes in the patient's condition.
• Document the patient's plan of care using identified nursing diagnoses, expected patient outcomes, and selected nursing interventions.
• Perform interventions according to identified priorities, plan of care, hospital policies, and patient care outcome standards.
• Revise the plan of care according to evaluation, changes in the medical plan of care, and effective/ineffective nursing interventions.
• Use clinical judgment in evaluation activities to meet patient care needs of an assigned unit/floor, including establishing priorities.

• Must be a licensed RN with a valid license.
• Familiarity with Federal, State, and local long‑term care requirements.
• Two years of professional nursing experience.

• Full‑time position.
• Monday‑Friday schedule, days only.
