Requisition

The Consult Coordinator/Telehealth Coordinator facilitates medical consultations and appointments for inmates requiring specialized care beyond the facility’s capabilities. The role involves coordinating with outside healthcare providers, scheduling appointments, arranging transportation, and ensuring necessary medical records are prepared. The Coordinator also monitors patients from intake to release, advocates for patient needs, and collaborates with multiple systems impacting offenders’ health.

• Meet with inmates/patients regarding complaints of medical care and facilitate resolutions with the patient and medical team.
• Coordinate medical consultations and telehealth appointments, including scheduling and transportation.
• Arrange for necessary medical records and prepare documentation for external providers.
• Track and trend data for presentation to a committee.
• Monitor patients from intake to release, ensuring continuity of care.
• Act as a liaison between the correctional facility’s medical team and external healthcare providers, communicating medical needs and coordinating information exchange.
• Maintain accurate and up-to-date medical records for patients receiving specialized care.
• Perform administrative tasks such as scheduling appointments and coordinating with external providers.

• Graduate of an accredited School of Nursing.
• Licensed as a Registered Nurse or Licensed Practical Nurse in the state of employment.
• Active CPR certification.
• Knowledgeable about state laws and regulations governing practice.
• Completed initial and annual clinical competencies as required by the role.
• Ability to work independently, take initiative, and exercise good judgment and problem-solving skills.
