Polaris Healthcare and Rehabilitation Center, a leading provider of healthcare services in Milford, Delaware, is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team! As an LPN at Polaris, you will be part of a dynamic and supportive clinical team that is dedicated to providing high-quality care to our residents in a warm and welcoming environment.
Responsibilities- Provide direct patient care under the supervision of a Registered Nurse (RN) or other licensed healthcare professionals
- Administer medications, treatments, and procedures as prescribed by healthcare providers
- Monitor and record vital signs, medical conditions, and treatment outcomes
- Assist with invasive and non-invasive procedures, including IV therapy and wound care
- Collaborate with other healthcare professionals to develop and implement individualized care plans
- Communicate effectively with patients, families, and healthcare team members to ensure seamless care transitions

- Active Delaware LPN License
:
Possess an active LPN license in the state of Delaware. - LTC Experience Preferred
