- REQUIRED:
Prior Long Term Care Experience
We provide skilled nursing care to people recently discharged from the hospital, needing therapy & to those needing long-term care.

- Care Needs & Planning:
Assist to implement & revise care plans, monitor health conditions, supplement direct care needs & respond to emergencies or changes in status. - Staff Oversight & Scheduling:
Observe care assignments, manage coverage needs, evaluate staff performance, educate & lead staff. - Family & Team Liaison:
Communicate changes in resident conditions to families/providers, work collaboratively with the interdisciplinary team. - Documentation:
Review and investigate incidents, transcribe orders, audit medical charges and maintain record compliance for accuracy. - Family, Resident, Staff
Education:
Explain health status, changes and expectations for care, guide and direct care skills with intervention as needed.
- Licensed to work within the State of Delaware as a RN Registered Nurse.
- Able to pass background check, drug screen, professional references check & physical evaluation.
- Excels in leadership and clinical competency.
