Licensed Practical Nurse (LPN)Position Summary
The Licensed Practical Nurse (LPN) is responsible for providing clinical oversight, coordination, and direct nursing interventions to support the health, safety, and well-being of program participants. This role ensures participants receive appropriate medical care through ongoing assessment, medication administration, health monitoring, and collaboration with interdisciplinary team members.

The LPN plays a critical role in identifying health concerns, responding to medical needs, maintaining accurate health records, and promoting compliance with clinical standards and best practices.
Key Responsibilities Direct Clinical Care- Provide direct nursing care and clinical support to program participants.
- Assess participant health status and respond promptly to medical concerns and changes in condition.
- Administer medications and treatments in accordance with physician orders, nursing standards, and organizational policies.
- Monitor participants for medication effectiveness, side effects, and health-related concerns.
- Provide immediate intervention during medical or health-related situations.
- Observe, report, and document participant health outcomes to support continuity of care.
- Communicate effectively with healthcare providers, program staff, families, and other stakeholders regarding participant health needs.
- Collaborate with interdisciplinary team members to ensure coordinated and person-centered care.
- Report health concerns, incidents, and updates to program leadership in a timely manner.
- Support implementation of health plans and physician recommendations.
- Maintain accurate, timely, and confidential medical records and documentation.
- Complete nursing notes, medication administration records (MARs), and incident reports as required.
- Monitor compliance with organizational policies, healthcare regulations, and clinical standards.
- Safeguard participant information and maintain strict confidentiality in accordance with HIPAA requirements.
- Assist with audits, inspections, and quality assurance activities as needed.
- Current and valid Licensed Practical Nurse (LPN) license in good standing.
- High School Diploma or GED required.
- Graduate of an accredited Practical Nursing program.
- Experience working with individuals with intellectual, developmental, behavioral health, or medical support needs preferred.
- Experience in community-based services, residential services, healthcare, or human services settings is a plus.
- Strong knowledge of nursing practices, medication administration, and healthcare procedures.
- Ability to assess participant needs and respond appropriately to medical concerns.
- Knowledge of infection control, safety procedures, and healthcare regulations.
- Ability to handle emergency situations with professionalism and sound judgment.
- Strong written and verbal communication skills.
- Proficiency in documentation, recordkeeping, and electronic health records.
- Ability to work independently while collaborating effectively within a multidisciplinary team.
Demonstrates strong nursing knowledge and clinical judgment to provide safe, effective, and participant-centered care.
