Salary

Annual salary up to $41,000.00 for Level I or $43,000.00 for Level II.

Schedule

Full‑time, Monday‑Friday with occasional weekends. Primary hours 8:00 AM‑5:00 PM, occasional evenings.

Position Description

The Community Health Worker is responsible for conducting outreach, community education, informal counseling, social support and advocacy to build individual and community capacity to facilitate access to services. Frontline public health workers act as liaisons between health and social services and the community to improve service delivery quality.

Responsibilities
  • Meet with clients to assist them in understanding diagnosis, after‑care instructions, referrals, insurance, and medication coverage.
  • Assist clients in applying for social service assistance and making referrals for housing, food, transportation, and other community resources.
  • Collaborate with clients to identify goals and next steps for overall wellbeing by building trust and maintaining active relationships during follow‑up contacts, including calls, home visits, and meetings.
  • Facilitate or conduct outreach, such as providing professional services, to meet clients within their normal environment and connect with the community.
  • Motivate clients to be active, engaged participants in their health.
  • Work effectively with people from diverse backgrounds to reduce cultural and socio‑economic barriers between clients and institutions.
  • Document all client contact in a prescribed manner.
  • Maintain Health Insurance Portability and Accountability Act (HIPAA) and agency standards regarding client confidentiality.
  • Advocate for clients by identifying barriers and increasing access to state services and community resources.
  • Collaborate with the team to provide the clinical team with findings and ensure coordinated care and referrals.
  • Serve as liaison between state agencies and local community programs to support public health and wellbeing, communicating health promotion and education efforts to varied audiences.
  • Participate in required training and professional development activities.
  • Being present in the office is an essential function of this job.
  • Other duties as assigned.

Minimum Qualifications Level I

One (1) year of health care, community involvement, and/or outreach experience (paid or volunteer) in the community.

Level II

A high school diploma or equivalent and two (2) years of health care, community involvement, or outreach experience serving the community.
