Primary ObjectiveThe Service and Support Administrator (SSA) will assist an individual to be self‑directed in accessing services and supports, integrate resources, ensure the rights of individuals, and remove barriers to improve quality of life.
Responsibilities- Act as primary point of coordination for individuals aged three and above.
- Annual engagement with the individual/family to develop and update the Ohio Individual Service Plan (OISP).
- Conduct ongoing assessment of current needs, including skill development, health, safety, and preferences.
- Monitor authorized services and review progress against OISP objectives.
- Follow up with service providers, facilitate communication, and address needs identified in reports.
- Amend OISP as needed based on progress, MUI prevention plans, or other issues.
- Refer and link to available resources.
- Document case‑management notes, billing, and Waiver administration.
- Conduct required assessments (Level of Care, Developmental Disability Profile, AAI, NTI, etc.) annually.
- Oversee waiver enrollment, re‑determination, and dis‑enrollment processes.
- Ensure provider choice process, tours, interviews, and credential reviews.
- Identify funding sources, establish budgets, and manage prior authorization and revisions.
- Report major incidents, including alleged abuse or neglect.
- Provide training and support to other Service and Support Administrators.
- Bachelor’s degree in social work, counseling, education, health and human services, law enforcement, behavioral science, or a related field.
- Master’s degree and at least two years of experience with individuals with developmental disabilities preferred.
- Must obtain and maintain DODD Service and Support Administration certification per OAC 5123‑5‑02.
- Valid Ohio driver’s license and acceptable driving record.
Light work; may require lifting up to 25 pounds. Manual dexterity, talking, hearing, reaching, and navigating the environment.
Other Requirements- Successful completion of criminal records checks (BCI, FBI) and registry/database check.
- Favorable reference reports from DCBDD.
- Participation in On‑Call rotation for after‑hours, weekends, and holidays; may need to intervene in crisis or authorize emergency support.
- Flexible schedule to meet operational needs.
- Adherence to ORC, OAC, and DCBDD policies and procedures.
- Regular attendance per regulations.
- Other related duties as assigned.
