Shift:Monday through Thursday (12p-10p) providing care and patient follow-up with RN/Case Manager or On-Call RN/Case Manager.

Description:Hospice LPN - Provide Hospice Nursing services offering emotional, social and spiritual care and support to patients and their care givers.
Scope of Work:- Assist the RN/Case Manager with providing hospice care in accordance with the patient's plan of care and performing specialized procedures as directed by attending physician and Hospice Medical Director.
- Submit complete, accurate and relevant clinical notes in a timely manner and assist in preparation, implementation, and continuing evaluation of the patient plan of care.
- Assist in providing and reinforcing education to patients/family members regarding the dying process and obtain medical history as needed.
- Use of Electronic Health Record, Email, computers, and cell phone devices.
- Graduate from an accredited School of Nursing.
- Licensed in the State of Ohio as a Licensed Practical Nurse.
- Valid, unencumbered LPN license and able to administer pharmaceuticals, perform medication reconciliation, and any nursing procedures or treatments required within the scope of nursing practice for the state.
- One year of LPN experience; hospice experience preferred.
- Excellent assessment and problem‑solving skills.
- Skillful in organization, time management and knowledge of symptom management.
- Licensed driver with a reliable, insured automobile.
- Able to communicate well, both verbally and in writing.
- Graduate from an accredited School of Nursing.
- State of Ohio License - Registered Nurse (Unencumbered).
