Job Overview
As a Tranquility Hospice Nurse (RN), you will deliver compassionate, patient‑centered care, coordinating the interdisciplinary team to meet the physical, psychosocial, medical, emotional, and spiritual needs of patients and their families.

ShiftMonday‑Friday 8:30 a.m. – 5 p.m. (flexible) with on‑call rotation
Responsibilities- Plan, coordinate, and provide hospice care in accordance with patient care plans.
- Collaborate with patients, families, and team members to ensure all needs are met.
- Consult with the attending physician and Hospice Medical Director regarding changes in patient condition that require order or plan modifications.
- Submit complete, accurate, and timely clinical notes, and participate in IDG and case conferences.
- Provide education to patients and families about the dying process, care plans, and healthcare decisions.
- Collect and document medical histories relevant to current condition.
- Serve as a case manager, implementing the nursing process and updating plans as needs change.
- Supervise and educate STNA’s and LPN’s.
- Participate in bi‑weekly interdisciplinary team meetings.
- Identify and utilize resources and referrals (e.g., OT, PT, Pharmacy, medical supplies, durable medical equipment).
- Communicate effectively through written and verbal shift reports.
- Use electronic health records, email, and other technology tools.
- Minimum of one year RN experience.
- Experience in hospice, home health, or long‑term care preferred.
- Strong leadership and organizational skills.
- Effective assessment and critical thinking abilities.
- Knowledge of case management processes.
- Valid driver’s license with reliable, insured automobile.
- Excellent verbal and written communication skills.
- Graduate of an accredited School of Nursing.
- State of Ohio Registered Nurse license (unencumbered).
- Physical activities include walking, sitting, stooping, and standing.
- Participate in on‑call rotation based on program needs.
- Travel to service counties as required.
- Can lift up to 50 pounds.
