Willow Brook Home is seeking a full-time MDS Coordinator to support our 50-bed care community, known for its steady leadership and collaborative clinical team.
In this role, you’ll take ownership of key clinical processes that require accuracy, consistency, and judgment:
- Completing MDS and CAA assessments thoroughly and on schedule
- Participating in and contributing to care plan conferences
- Developing and refining individualized nursing care plans based on resident needs
- Working closely with the interdisciplinary team to support quality outcomes
- Stepping in as a staff nurse as needed to stay connected to resident care
We are specifically looking for candidates who bring:
- Strong, hands‑on experience with MDS and Medicare reimbursement (preferred and highly valued)
- Active RN license in Ohio
- Confidence in documentation, timelines, and regulatory expectations
- Clear communication and a detail‑oriented mindset
