A skilled nursing facility in Lewes, DE is seeking a compassionate, state-certified social worker to join its Social Services team on a part-time basis. This is a meaningful role for someone who enjoys supporting residents and families through every step of their care.

• Must be a state-certified LSW or CSW

• Complete psychosocial assessments and participate in care planning
• Support residents and families with adjustment, concerns, and emotional needs
• Coordinate discharge planning and connect residents with community resources

• Maintain accurate, timely documentation in line with state and federal regulations

• Active LSW or CSW certification
• Long-term care or skilled nursing experience preferred
