Lifespace Communities - 1717 Homewood Boulevard - Responsibilities:
Follow care plan directives and administer direct patient care to residents;
Assist residents with daily living activities (feeding, bathing, dressing, grooming, transferring);
Record and report changes in residents' conditions to the nurse and coordinate with nursing team;
Monitor and record vital signs and ADLs; maintain accurate documentation;
Maintain infection control standards and a safe, clean environment; respond to call lights promptly
