Family System Navigator – Mobile Response Team, Palm Beach County
Full-time.
This role at South County Mental Health Center, Inc. – a private, non‑profit Behavioral Healthcare provider – aims to decrease fragmentation of care by coordinating services and guiding families through the child welfare system. Navigators engage in outreach, education, and partnership building to support families and system partners.
Essential Job Functions- Develop and maintain collaborative relationships with system partners.
- Serve as liaison and navigation coordinator for partners such as Child Welfare, the Department of Juvenile Justice, and School Districts.
- Coordinate with local law enforcement, hospitals/Baker Act facilities, civil diversion programs, and other relevant agencies.
- Provide outreach to partners regarding Mobile Response Services, developing Memorandums of Understanding and Collaborative Agreements.
- Educate partners on the benefits and appropriate use of Mobile Response Services.
- Attend system partner meetings where MRT representation is needed (e.g., SHAC‑Mental Health, school safety, Child Welfare/Behavioral Health Integration).
- Offer technical assistance to MRT staff on Child Welfare, School Systems, and Juvenile Justice.
- Collaborate with Child Welfare to assist foster parents in preventing placement disruptions.
- Contribute to MRT policies and procedures when appropriate.
- Provide training and coaching to MRT staff on system navigation and needs.
- Act as liaison between Mobile Response and other system partners to ensure effective communication and meet needs.
- Bachelor’s degree in a human service‑related field.
- Minimum 5 years of experience in children’s systems.
- Strong working knowledge of Child Welfare, School Systems, and Juvenile Justice.
Pay ranges from $22.00 to $27.00 per hour. Full‑time employees receive a comprehensive benefits package including health, dental, vision, paid time off, and a 401(k) with matching contributions.
