As a Coach, you will enable practice teams to adopt improvements aligned with the Patient Medical Home model. Collaborating closely with various stakeholders, you’ll customize approaches to support implementation while developing quality improvement capacity amongst teams in primary care settings.
Key Responsibilities:
• Lead improvements in primary care settings
• Foster quality improvement capacity in practice teams
• Engage with community partners and health authorities
• Evaluate performance trends and suggest enhancements
• Communicate effectively with client groups and stakeholders
Requirements:
• Bachelor's degree and 3-5 years of experience
• Strong understanding of BC's health system
• Experience in quality improvement methods in healthcare
• Ability to apply coaching techniques for team development
• Skilled in facilitating small group sessions effectively
