Location: Deltona, Florida
Status: Full-Time
Compensation: $40–$46 per hour (based on experience)
Position SummaryWest Volusia Healthcare & Rehabilitation is seeking an experienced and highly organized MDS Director (RN/LPN) to lead and oversee the MDS process while ensuring regulatory compliance and high-quality, resident-centered care. This key leadership role supports accurate care planning, clinical excellence, and strong interdisciplinary collaboration.
Key Responsibilities- Oversee the accuracy, completeness, and timeliness of all MDS assessments and documentation
- Ensure compliance with state and federal regulations, including Medicare and PDPM requirements
- Conduct comprehensive resident assessments and interviews
- Develop, implement, and monitor individualized care plans for short- and long-term residents
- Lead and participate in interdisciplinary team meetings
- Collaborate with nursing staff, caregivers, physicians, specialists, and families to optimize resident outcomes
- Provide leadership, education, and ongoing support to nursing staff
- Conduct in-service training related to MDS, RAPS, Triggers, PDPM, and Medicare guidelines
- Maintain MDS and care plan schedules for both Medicare and non-Medicare residents
- License: Active Florida RN or LPN license in good standing
- Experience: Minimum of 2 years of MDS Coordinator experience in a Skilled Nursing Facility
- Knowledge: Strong working knowledge of MDS, PDPM, Medicare guidelines, and care-planning processes
- Skills: Excellent organizational, leadership, and communication skills
