Location: Deltona, FL
Position: Admissions Coordinator
Schedule: Full-Time
Salary: $31.25 to $40.86 hourly based on experience
West Volusia Healthcare & Rehabilitation is seeking a motivated, detail-oriented, and compassionate Admissions Coordinator to join our growing team. In this key role, you will serve as the primary liaison between hospitals, referral sources, residents, and families, ensuring a seamless admissions process while helping drive occupancy growth and exceptional customer service.
Key Responsibilities- Coordinate and facilitate the admissions process from referral through admission.
- Collaborate with hospitals, physicians, case managers, and referral sources to ensure smooth resident transitions.
- Develop and maintain strong relationships with referral partners and community organizations.
- Support marketing and outreach efforts by attending networking events and community functions.
- Educate prospective residents and families regarding services, policies, visitor guidelines, and admission procedures.
- Verify insurance coverage, financial information, and authorization requirements prior to admission.
- Maintain working knowledge of Medicare, Medicaid, Managed Care, and payer-specific guidelines.
- Monitor admissions activity and prepare monthly reports on admissions trends and census growth.
- Identify operational barriers affecting admissions and work with leadership to implement solutions.
- Communicate admission details effectively with interdisciplinary team members to ensure continuity of care.
- Minimum of 2 years of admissions experience in a Skilled Nursing Facility (SNF) required.
- Strong understanding of Medicare, Medicaid, Managed Care, PDPM, HMO guidelines, and RUGS II & III.
- Experience working with Electronic Medical Record (EMR) systems.
- Knowledge of CMS regulations and Department of Health (DOH) requirements.
- Excellent communication, organizational, and relationship-building skills.
- Ability to multitask and work effectively in a fast-paced healthcare environment.
