Shift: 8

Hrs. Regular Days Shift

Scheduled Hours: 40 Hours a Week

Hourly Pay: $53.00 - $60.00

• Work with providers and clinic staff to identify patients with chronic disease diagnoses.
• Support longitudinal care of patients with stable chronic conditions through focused assessments.
• Conduct motivational interviews and set self-management goals.
• Create referrals to appropriate agencies and resources.
• Support the transition of patients with stable chronic conditions from inpatient to outpatient settings.
• Perform focused assessments of transitional needs.
• Assist with post-discharge needs such as prescriptions, transportation, DME, and appointments.
• Notify providers and/or supervising RN of changes to patient’s health condition.
• Ensure accurate and timely documentation in the electronic medical record.
• Participate in daily huddles and care coordination meetings.
• Prepare reports for weekly meetings.
• Maintain an active LPN License for FL or a Compact Nurse license.
• Hold a current BLS certification.
• Have at least one year of experience.
• Previous experience in a Primary Care setting is highly preferred.
• Experience in case management or care coordination is required.
