Luna Rehab & Care Center is seeking a Part-Time or PRN (consistency) Occupational Therapist to join our in-house rehabilitation team. Position OverviewAs an Occupational Therapist (OT), you will evaluate, plan, and deliver individualized treatment using evidence-based approaches to help residents regain independence and improve quality of life. You’ll also play an essential role in patient and family education, interdisciplinary collaboration, and clinical documentation.
Key Responsibilities- Evaluate residents to identify functional limitations and therapy needs
- Develop and implement personalized treatment plans focused on ADLs and IADLs
- Monitor progress and adjust interventions in collaboration with the care team
- Recommend adaptive equipment and environmental modifications
- Educate patients, families, and caregivers on strategies for functional success
- Maintain accurate and timely documentation per regulatory standards
- Participate in care planning and interdisciplinary meetings
- Ensure compliance with all facility and regulatory guidelines
- Active Occupational Therapist license (NM)
- In good standing with all licensing boards
- SNF or long-term care experience preferred
- Experience with Net Health documentation system is a plus
- Strong communication and interpersonal skills
