Description
An Emergency Department Patient Navigator is a trained, culturally sensitive health care worker capable of providing support and guidance helping patients transition from the emergency department to outpatient follow-up care or home. Provide support and guidance throughout the hospital and community. Help navigate doctors' offices, clinics, hospitals, outpatient centers, insurance and payment systems, and other components.

- Assisting with SBIRT Screening
- MH Care Coordination
- Monitoring and following up on discharge call backs
- Coordinating appointments with providers to ensure timely delivery of diagnostic and treatment services.
- High School Graduate or GED
2 years experience in healthcare, hospital or clinic environment.
Experience in customer relations preferred
- Ability to organize and manage a large volume of incoming documents accurately with minimum supervision
- Ability to work cooperatively with others
- Capable of data entry and productivity utilizing Microsoft Office Suite (Word, Excel, PP, etc)
- Detail oriented
- Able to work well under heavy workload
- Optimistic, energetic and outgoing
