Carry out patient's established plan of care using therapeutic modalities and techniques;
Complete documentation related to care provided, communicating to supervising therapist and team members on a timely basis;
Assist in mobility, transfer, and therapeutic exercises as assigned;
Maintain professional interaction with patients and team; support cross-facility coordination;
Provide support to physical therapy team to ensure continuity of care across facilities
