• Conduct comprehensive Physical Therapy assessments for referred patients, considering physiological, psychosocial, environmental factors, and health history.
• Integrate assessment data to develop and implement individualized plans of care that promote patient function and safety.
• Maintain annual HHRG (Home Health Resource Group) values at or above national standards and use these values to guide visit frequency and interdisciplinary utilization.
• Achieve annual patient outcomes that meet or exceed national benchmarks.
• Refer patients to appropriate community resources as needed.
• Evaluate clinical needs, initiate and revise patient care plans, and document all problems and interventions accordingly.
• Participate in interdisciplinary patient case conferences to evaluate and adjust care plans.
• Supervise and instruct Home Health Aides, ensuring documented oversight every 14 days.
• Notify physicians and other care providers promptly about significant changes in patient conditions.
• Complete all admission, clinical, and discharge paperwork accurately and on time.
• Ensure clear, concise documentation reflecting skilled care, teaching, direct care, and patient/caregiver responses.
• Obtain and clarify valid physician orders for all care provided.
• Plan and coordinate patient discharges, communicate with patients and physicians, and complete all discharge documentation.
• Consistently demonstrate service excellence in alignment with the organization's CARES values: compassion, accountability, respect, excellence, and service.
