Job Description - Dietitian, (Per Diem), VNA of Cape Cod

Purpose of Position :
Provides nutritional consultation to both staff and patients consistent with professional standards. Participates with other personnel in the development of agency policies and programs necessary to achieve agency objectives as they relate to patient care.

• Provides consultation to agency staff concerning nutritional problems/ concerns of the Agency population.
• Provides in-service educational programs for the staff of the agency as required.
• Perform patient assessment and evaluation when requested by the physician or staff clinician.
• Develops a plan of care for each patient receiving nutritional consultation, and in cooperation with the patient’s physician.
• Performs complete and timely record keeping according to agency policy for purposes of documenting accountability of care given, for continuity of patient care, for planning and evaluating.
• Develops and updates a resource file for reference and nutrition education handouts.
• All other duties as assigned.

Consistently provides service excellence to all patients, family members, visitors, volunteers and co-workers in a manner that reflects VNA’s commitment to CARES: compassion, accountability, respect, excellence and service.

• Bachelor of Science degree in Dietetics from an accredited school of Dietary Science.
• Completed appropriate internship program and/or Master’s Degree.
• Current Mass. Dietitians/ Nutritionist (LDN) License.
• Current registration with the American Dietetic Association required.
