POSITION SUMMARY
RN Case Manager – Hospice
POSITION DUTIESProvides skilled nursing care, supportive care, and pain and symptom management to maximize comfort and quality of life for Hospice patients and their families. Provides care to patients in both homes and facilities.
MINIMUM REQUIRED QUALIFICATIONS- MA RN license
- 1 year of nursing experience or 6 months of nursing experience with a BSN
- BSN
- Hospice Home Care experience, IV Certification, Computer skills
13 weeks
SHIFT / HOURS PER WEEK8am-4pm, - weekday, weekend, holiday rotation
START DATE9/28/2026
