Position: LICENSED PRACTICAL NURSE

Maplewood Brewster LLC is seeking a compassionate Wellness Nurse (RN/LPN) to join our Healthcare & Medical Services team in our assisted living and memory care community. In this role, you will assess residents, administer medications and treatments, coordinate individualized care plans, and collaborate closely with physicians, families, and care staff. You'll support resident well-being through health education, monitoring changes in condition, and promoting safety.

Responsibilities

  • Provide direct nursing care to residents, including assessments, treatments, and medication administration according to orders and facility policies.
  • Monitor residents' health status, recognize changes in condition, and initiate appropriate interventions and follow-up with providers and families.
  • Develop, implement, and regularly update individualized wellness and care plans in collaboration with the interdisciplinary team.
  • Document assessments, interventions, and outcomes accurately and timely in electronic medical records.
  • Support residents with memory impairment using person-centered approaches that promote dignity, safety, and emotional well-being.
  • Educate residents and families about care plans, medications, disease processes, and wellness strategies.
  • Collaborate with caregivers, activities staff, and leadership to ensure a consistent, resident-centered approach to daily care and engagement.
  • Adhere to infection control, safety, and regulatory standards, contributing to audits, quality initiatives, and survey readiness.
  • Provide clinical guidance and support to care team members, helping to prioritize care and maintain high standards of practice.
  • Participate in training, in-services, and professional development opportunities to enhance clinical skills and knowledge of geriatric and memory care nursing.
