Qualified Intellectual Disability Professional III Job Title: Qual Intel Disability Prof III

Responsibilities
  • Lead and coordinate the Interdisciplinary Team (IDT) and oversee the Individual Support Plan (ISP) process for all individuals on the assigned caseload, including ISP Preparation, ISP Addendum, and Annual ISP meetings.
  • Ensure assessments and evaluations are conducted to determine the current level of functioning, preferences, strengths, needs, and recommendations, and document outcomes.
  • Collaborate with the individual and Legally Authorized Representative (LAR), paraprofessional staff, and professional/clinical staff in developing and delivering needed supports and services.
  • Follow up on recommendations for supports, services, equipment, or programs to ensure timely provision of needed services and supplies.
  • Monitor and document ISP implementation and effectiveness; coordinate with the IDT for modifications based on progress or changes in behavioral or health status.
  • Facilitate acquisition of skills and adaptive behaviors and promote competent staff interactions in program implementation and behavior management.
  • Ensure compliance with federal and state standards governing ICF/IID, the Settlement Agreement with the DOJ, SSLC policies, and procedures; maintain appropriate documentation in electronic records systems.
  • Work with individuals who have complex needs and may exhibit aggressive or self-injurious behaviors; balance priorities and demonstrate strong organizational and scheduling skills.
  • Provide training and coaching to staff on service delivery, supports, and teaching methodologies; lead ad hoc training and serve as a role model for staff at multiple levels.
  • Assist in training, coaching, and mentoring of other QIDP staff as needed; coordinate with behavioral health staff to integrate behavioral supports into the ISP.
  • Monitor monthly ISP plans, programs, supports, and services; document analysis and provide recommendations for interventions or revisions as needed.
  • Ensure client protections, follow Human Rights Committee (HRC) processes for restrictive practices, and advocate for the individual.
  • Maintain documentation across disciplines in an electronic records system; meet all required deadlines per ICF/IID standards.
  • Participate in disaster response, COOP activation, or other agency obligations as assigned, which may require alternate shift patterns or locations.
Knowledge,

Skills And Abilities

(KSAs)
  • Knowledge of the IDT process, people-centered planning, rehabilitation and skills acquisition programs for individuals with intellectual disabilities, program design, and data analysis.
  • Knowledge of or ability to develop knowledge of discipline-specific plans such as PBSPs and PNMPs.
  • Ability to coordinate programmatic and clinical services; prepare and maintain records; communicate clearly in writing and verbally.
  • Ability to assess and solve problems; work independently and prioritize tasks; adapt quickly to change; work harmoniously with staff and LARs.
  • Ability to perform direct care work as needed, including emergency procedures.
  • Facilitation skills for IDT meetings and proficiency with personal computers and electronic records systems.
Qualifications
  • Initial Screening Criteria: Bachelor’s degree from an accredited college or university with a major in psychology, sociology, social work, special education, rehabilitation counseling, or related human services field, or equivalent combination of subject matter.
