The AMH Team Leadworks under the leadership of the Program Manager of Adult Mental Health andwithin federal, state and local guidelines performs case management and rehab services with adults, as well as team lead duties.??

Work involves developing and maintaining long-term contact with clients, client families/LAR, and service providers for medical, social, educational, and related service needs.

This position is responsible for overseeing the care of adults diagnosed with a mental illness and assigned to various Levels of Care (LOC). This position ensures continuity of care for individuals. Team lead duties include training new staff on policies and procedures, workflows, day-to-day job functions and responsibilities, reviewing and auditing staff notes, scheduling one on one orgroup training activities, review audit CAPS and follow up with staff, and providing guidance and support to team.

This position works under moderate supervision, with limited latitude for the use of initiative and independent judgment.

Education,Training, and Experience
  • Education:

    Graduation from an accredited four-year college or university with major coursework in human services field as required to qualify as a Qualified Mental Health Professional (QMHP), required.
  • Master's Degree in a human services field, preferred.
  • Previous Crisis experience preferred.
  • Experience:

    Six (6) months prior experience in a mental health field preferred.
Registration,Certification, Licensure, and Other Qualifications
  • Must haveand maintain a background and criminal history free from any disqualifying offenses as outlined by the Texas Administrative Code (TAC) and the Healthand Human Services Commission (HHSC).
  • Must possessand maintain a valid driver's license and automobile insurance.

oIndividualswith an out-of-state driver's license must be able to obtain a driver's licensein the state of Texas within thirty (30) days.

Successful completion of all position-specific training within thirty (30) days of employment is required.

Knowledge,Skills, and Abilities
  • Knowledge of community resources, of case management delivery systems, and of program policies and procedures.

    Basic knowledge of Microsoft Word, Excel, and PowerPoint.
  • Skill in active listening, de-escalation techniques, empathy, and basic knowledge of social services systems. Skill documenting services provided, conducting assessments, and developing recovery plans. And skill in communicating with a variety of individuals from various backgrounds and experiences.
  • Ability to:

oassess clients' needs, coordinate individuals'services, communicate effectively, and to train others.

owork as a member of an interdisciplinary teamto ensure referral to appropriate Center program as needed.

owork independently and make decisions butseeks guidance when needed.

omake decisions based on education and or clinical experience, which reflect consistency with Center's Guiding Principles, Policies and Procedures, and Standards of Care.

owork with clients of different cultural backgrounds,sexual orientation, and belief systems.

owork with clients who may display symptoms ofmental illness including; poor hygiene, difficulty controlling anger and low frustration tolerance, difficulty or inability to display appropriate social behavior, and who are suicidal or homicidal.

oThe ability to engage with individuals servedin community locations, including but not limited to residences, community businesses, and local hospitals.

  • Understanding and practice of Trauma Informed Care principles.

1.Work with Program Manager to develop, schedule, and run team meetings.

2.Staff will develop team meeting agendas to include important Center updates as well as addressing any training/education deficiencies or reviewing new policies and procedures.

3.Staff will createand maintain a sign-in sheet for other staff to sign in and acknowledge participation and understanding.

4.If a meeting ismissed by a team member, staff will get with them within one week to review the information.

5.Provide courteous, professional services at all times.

6.Handle calls from clients or others related to complaints or problems/concerns with services or treatment and contacts the Program Manager and Rights Department where needed.

8.Interact with Center staff, individuals, and community members in a professional manner.
