Job Description
Support individuals with intellectual and developmental disabilities who are medically fragile or behaviorally complex in a campus-based setting. Perform advanced vocational nursing work, including complex clinical care, training, and oversight of nursing staff.
ResponsibilitiesPerforms highly advanced vocational nursing work. Provides care, treatment and general welfare to individuals residing at the State Supported Living Center with complex medical, mental health and/or behavioral health issues. Trains, models, coaches and oversees the work of other LVNs during the course of performing own duties. Performs basic and advanced routine and non‑routine nursing tasks to promote health of assigned individuals and shift.
Collects specimens for laboratory work and performs health screenings. Instructs direct support professionals and other staff on how to provide service and care that supports the individual’s medical needs. Performs technical nursing procedures per facility and state regulations. Keeps informed of and follows all agency, SSLC and unit/home policies and procedures. Provides direction and assists with on‑the‑job training to others such as newly hired LVNs and direct care staff.
Provides on‑the‑job nursing instruction and training for new LVNs within their scope of practice as requested by RN Supervisor. Monitors, oversees the work of, and assigns work to, LVNs as needed. Evaluates nursing intervention outcomes and provides input to other members of the healthcare team. Assists RNs with implementation of protocols, procedures and programs to improve overall health and safety of staff and individuals.
Works under the limited supervision of a Registered Nurse (RN), with latitude for independent judgment.
- Attend work on a regular basis and may be required to work specific shift schedule or a rotating schedule, extended shift and/or overtime in accordance with agency leave policy.
- Perform complex and technical nursing procedures such as tracheostomy care, gastrostomy/jejunostomy care, wound care, breathing treatments. Collect specimens for laboratory examination and conduct appropriate screening tests when requested by lab personnel.
- Administer medications and treatments orally, rectally, internally or by injection. Make rounds and/or home visits as needed and work in other locations at the SSLC as required.
- Monitor individuals’ weight, diet, feedings and vital signs. Monitor individuals’ health status after treatments and procedures, recognizing signs and symptoms of illness and reporting accurately to RN and/or Primary Care Provider(s).
- Lead, monitor and oversee non‑nursing staff who are monitoring individuals before, during and after treatments and procedures. Instruct direct support professionals on how to monitor individuals after a treatment or procedure using instruction sheets.
- Evaluate nursing intervention outcomes and provide input to other members of the healthcare team, fostering interdisciplinary collaboration. Communicate pertinent information orally and in writing to other team members. Collect data for RNs and/or Primary Care Provider(s).
- Document and report pertinent treatments and observations in the individual’s medical record (electronic health record or paper chart) in accordance with SSLC procedures and protocols. Teach and coach other LVNs on how to perform documentation.
- Train, instruct and oversee new LVN staff to perform nursing functions at the SSLC, including acclimating them to the individuals, their needs and preferences. Provide feedback to supervisor and assist new LVN with the on‑the‑job training checklist.
