Job Title and Location
Admissions RN – Home Health, Denton, Texas.
Primary FunctionsPerform admissions and administer nursing care for patients of all ages in their place of residence. Coordinate care with the interdisciplinary group, patient/family, and referring agency, and assume responsibility for coordination of care.
Responsibilities- Works in patients' homes in various conditions and locations.
- Assess home‑health patients utilizing the Nursing Assessment and LCD criteria to identify physical, psychosocial, and environmental needs.
- Ability to work a flexible schedule; ability to travel locally and respond to PRN emergency calls.
- Develop and document the plan of care in correlation with the assessment.
- Provide nursing care utilizing infection control measures (OSHA).
- Work with the interdisciplinary group to ensure patient and family needs are met.
- Communicate with attending physicians, home‑health physicians, and other physicians involved in the patient’s care.
- Submit all required paperwork in a timely manner.
- Demonstrate commitment, professional growth, and competency.
- Meet productivity standards.
- Perform on‑call responsibilities and provide on‑call service to patients and families as assigned.
- Provide effective communication to patients/families, team members, and other healthcare professionals.
- Education:
Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing. - Licensure:
Current state license as a Registered Nurse; proof of current CPR certification; current driver’s license. - Experience:
Two years as a Registered Nurse in a clinical setting; one year full‑time home‑health experience preferred but not required. - Knowledge of OASIS highly preferred but not required.
- Skills:
Strong assessment and clinical skills; excellent communication skills; ability to work independently and as part of a team; familiarity with infection control measures (OSHA); knowledge of state and federal regulations; good interpersonal skills. - Home Care Home Base (HCHB) knowledge highly preferred.
