Responsibilities
The Discharge Planner is responsible for planning and coordinating activities related to discharge planning. The role includes developing and implementing a plan of care designed to meet the patient’s post‑hospitalization needs while ensuring continuity of care. The position does not involve supervisory responsibilities.

Job Duties- Communicate with referral sources within the first 24 hours of admission.
- With assistance from the Care Center, social workers, and nursing, identify all outside professionals currently working with the patient so that appropriate discharge plans can be completed and facilitate communication to minimize duplication.
- Identify needs of referral sources and/or professionals during the period of hospitalization.
- Oversee continuous communication of treatment progress, discharge plans, and time of discharge to outside interested parties, and facilitate the transfer of required documents to outside agents.

Education: Bachelor’s degree from an accredited college or university, preferably in social work or a clinical related mental health field.
Experience: Minimum of one year of experience in related disciplines with hospital experience strongly preferred. License: N/A.
Additional Requirements: Handle With Care training within 30 days of employment and prior to assisting in a restraining procedure. May be required to work flexible hours and overtime.
