The QIDP III plays a central role in overseeing the Individual Support Plan (ISP) process for residents at a State Supported Living Center. The position coordinates, monitors, documents, and follows up on all programs, services, and plans for the assigned caseload, leading and directing the interdisciplinary team (IDT). The role ensures compliance with federal and state standards, resolves program inconsistencies, and promotes behavioral management and skill acquisition for residents.

• Attend work on a regular basis and meet shift and scheduling requirements, including overtime and on‑call duties as needed.
• Orchestrate and oversee the ISP process, facilitating IDT meetings throughout the year.
• Ensure residents receive assessments and evaluations to determine their current functioning level, preferences, and recommendations.
• Coordinate participation of residents, their legally authorized representatives, and staff in planning and delivering supports.
• Follow up on recommendations for supports, services, equipment, or programs to meet residents’ needs.
• Observe and interact with residents across settings to monitor progress.
• Collect, analyze, and document ISP implementation data, proposing modifications when required.
• Provide training, coaching, and mentoring to staff on implementation of services and behavior management.
• Advise the interdisciplinary team on behavioral needs, integrating them into the ISP.
• Monitor and evaluate ISP plans and documentation on a monthly basis, reporting findings and recommendations.
• Respond promptly to life events and ensure client protections, following Human Rights Committee procedures for restrictive practices.
• Maintain accurate electronic records in compliance with ICF/IID standards.
• Participate in disaster response or continuity of operations activities as assigned.

• Monthly monitor ISP implementation and produce review documents.
• Protect residents through prompt response to life events and restrictive practices.
• Maintain electronic documentation in compliance with standards.
• Participate in COOP or disaster response as required.

• In-depth knowledge of the IDT process, people‑centered planning, and rehabilitation programs for individuals with intellectual disabilities.
• Knowledge of Positive Behavior Support Plans and Physical Nutritional Management Plans.
• Ability to coordinate programmatic and clinical services, prepare and maintain records, and communicate clearly.
• Strong problem‑solving, independent work, and ability to prioritize tasks.
• Ability to adapt quickly to change and work harmoniously with staff and legally authorized representatives.
• Physical capability to perform direct care tasks, including emergency procedures.
• Skilled in facilitating IDT meetings, using a computer to create documents and reports, and navigating electronic records systems.
