Job Description:
Monitors patients on medications with reported therapeutic levels or anticipated adverse effects. Maintains current clinical knowledge through guidelines implementation, evaluation, and fostering research. Other duties as assigned. Provides Collaborative Drug Therapy Management under physician protocols, including pain consultations, pharmacokinetics, anticoagulation, and renal adjustments. Reviews dosing, pharmacokinetics, and protocol compliance on medication regimes to ensure optimal therapeutic outcomes. Identifies and implements drug therapy for positive pharmacoeconomic impact, aiming to develop cost-effective clinical pathways for oncology patients.
Collaborates with the financial services team to ensure correct coding and documentation of medication administration for optimal financial impact. Monitors and analyzes medication purchasing reports to meet cost containment targets and contract compliance. Attends and presents relevant drug therapies at the hospital Pharmacy and Therapeutics Committee. Develops and conducts target drug programs and drug usage evaluations, reporting results to the committee.
Documents clinical interventions and cost avoidance in the pharmacy departmental program. Monitors the use of expensive medications for consistency with approved criteria. Anticipates changes in drug therapy and reports potential impacts on the hospital and drug budget. Promotes patient education on co-morbid conditions and oncology pharmacotherapy.

Education:
* Doctorate Pharm.
D degree [Required]
Field of Study:
* Completed a Pharmacy Practice Residency with experience in Oncology and/or Critical Care (PGY-1) and /or equivalent experience. A specialized clinical residency in Oncology (PGY-2) and /or experience is preferred. Board Certification(BCOPS) preferred
Work Experience:
* 1+ year experience as a clinical pharmacist or with equivalent education (completion of residency or Pharm.
D degree) [Required]
* Chemotherapy/Hazardous product compounding and USP 797 compliance [Required]
