What you will do:

We are looking for a Care Coordination Program Manager who can help shape our vision and support our mission. Here is what the position will look like.

  • Builds and maintains relations with medical and social organizations such as hospitals, medical, behavioral health and specialty providers, community-based organizations, county government programs, school districts, Case Management Agencies (CMA’s), HCPF, and local Department of Human Services.
  • Proactively partners with key stakeholders to support resolution of outstanding members or care team needs.
  • Leverages subject matter expertise to implement and adjust Care Coordination programs to ensure contract and deliverable success for Creative/Complex Services Solutions, Colorado System of Care (CO-SOC), high acuity youth including Children and Youth with Special Healthcare Needs, high acuity adult populations, and other identified member populations.
  • Responsible for contributing to HCPF contract deliverables which require collecting, editing, analyzing, and sending information to report owners and/or to HCPF directly. This specific program manager provides HCPF with status updates of high acuity members experiencing barriers to treatment, discharge, or finding appropriate care.
  • Responsible for managing the Enhanced Standardized Assessment (ESA) process internally, which requires receiving, reviewing, responding to and processing ESA requests. Additionally, this program manager will identify an in-network provider to accept the ESA. This program manager will also be responsible for tracking timeliness standards, making notifications as needed when a timeliness standard is at risk of being missed, and supporting the process to request an extension as needed.

    This program manager will also receive the completed ESA and route appropriately both internally and externally. This program manager will enter data as required by HCPF for tracking ESA and other identified CO-SOC metrics.
  • Effectively tracks, collates, analyzes qualitative and quantitative data, and provides reports to the executive leadership team and other key stakeholders to support care coordination program metrics and department performance.
  • Provides organizational contributions to key Care Coordination initiatives and programs.
  • Develops best practices in member referral and coordination between medical/non‑medical organizations and COA. This includes developing external facing training and materials to enhance referral and coordination practices.
