Social Worker​/Case Manager

The Center at Lowry is recruiting for a Case Manager to assist with patient placement referrals, discharge planning, assessments, scanning patient records, audits and other duties as assigned.

This is a full time position eligible for medical, dental, vision and supplemental insurances, paid time off and 401(k) with match.

Successful completion of a background check.

Must possess, as a minimum, a Social Service Designee Certificate and/or Bachelor Degree from an approved school of Social Work.

Must have, as a minimum, one year(s) experience in a hospital, skilled nursing care facility, or other related medical facility.

• Must be able to read, write, speak, and understand the English language.
• Must possess the ability to make independent decisions when circumstances warrant such actions.
• Must possess the ability to deal tactfully with personnel, residents, and family members, visitors, government agencies/personnel and the general public.
• Must possess the ability and willingness to work harmoniously with and supervise other personnel.
• Must have patience, tact, cheerful disposition and enthusiasm, as well as be willing to handle residents based on whatever maturity level in which they are currently functioning.
