CMHHiFL is one of two state hospitals serving our community. Its vision is to provide the highest quality mental health services to persons with complex, serious, and persistent mental illness. The mission is to support those individuals in achieving their recovery goals and reaching their full potential. The hospital offers inpatient treatment to adult patients (ages 18‑59) with 160 adult inpatient beds across more than seven treatment milieus.

Medical services, occupational therapy, neuropsychological‑rehabilitation services, and trauma‑informed care are available for all patients.

Role Summary – Social Worker II

The Social Work II position works in a 24‑hour state psychiatric hospital with individuals voluntarily and involuntarily committed for treatment. Responsibilities include clinical assessment, treatment planning, individual therapy, group therapy, family therapy, counseling, education, and coordination of discharge and after‑care plans. The Social Worker collaborates with the multidisciplinary team, community mental health centers, and other stakeholders to ensure continuity of care.

Responsibilities
  • Case Management and Discharge Planning
    • Manage own caseload and coordinate resources, including benefits, guardianship, referrals, and identification.
    • Work with Benefits Specialist and Reintegration Specialist to complete and monitor documentation.
    • Identify appropriate discharge locations and liaise with placements to support continuity of care.
    • Coordinate day passes, off‑campus appointments, and family visits.
    • Attend interagency staff meetings and provide education to stakeholders.
    • Collaborate with supervisor to navigate systemic barriers and support discharge into the least restrictive environment.
  • Assessment, Intervention, and Treatment Planning
    • Provide individual, group, and family therapy as determined by treatment plan meetings.
    • Develop group materials and lead sessions tailored to patient needs.
    • Communicate client treatment needs and barriers to support systems throughout the admission cycle.
    • Offer psychoeducation to support systems and community stakeholders.
    • Monitor effectiveness of interventions, lead treatment planning meetings, and ensure SMART goals.
    • Conduct biopsychosocial assessments within designated time frames, ensuring confidentiality and reliability.
    • Complete the Colorado Client Assessment Record (CCAR) and share information with the treatment team.
  • Other Duties as Assigned
    • Participate in hospital‑wide ad hoc activities, milieu support, and other assigned tasks.
Qualifications
  • Current, valid Colorado license or registered candidate as a Licensed Social Worker (LSW), Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor Candidate (LPCC), Licensed Marriage & Family Therapist Candidate (LMFTC), or Addiction Counselor Candidate (ADDC).
  • Full license from Colorado in any of the above fields also meets requirements.
Preferred Qualifications
  • Experience in case management/transition planning and clinical skills (individual and group therapy).
  • Social work degree.
  • Inpatient psychiatric treatment experience.
  • Treatment for adults with severe and persistent mental illness.
Conditions of Employment
  • May be exposed to bodily fluids or caustic chemicals.
  • Full background check required (CBI & FBI fingerprint check, Colorado Court Database check, Medicare Fraud Database, TRAILS Database check).
  • Direct care of vulnerable adults may require a CAPS check.
  • Motor vehicle check if employee will drive a state‑owned vehicle.
  • Valid, non‑restricted Colorado Driver’s License required; out‑of‑state holders must obtain a Colorado license within 30 days of employment start date.
