Registered Nurse - Telephonic

Salary: $80,000-$90,000 (negotiable, depending on experience) plus full benefits package

Job Summary:
Our client is seeking an experienced Registered Nurse (RN) to oversee care delivery for Principal Care Management (PCM) and Chronic Care Management (CCM) programs. This fully remote role focuses on supervising LPNs, signing off on care plans, and ensuring high-quality, compliant care for patients with neurodegenerative conditions such as Alzheimer's.

Job Responsibilities:

• Supervise LPNs providing care coordination and patient engagement
• Create, review, and approve individualized care plans
• Ensure Medicare PCM/CCM documentation compliance
• Conduct clinical quality assurance reviews
• Serve as liaison with neurologists and clinical partners
• Support training, mentorship, and workflow improvement
• Maintain accurate, time-based activity logs for billing compliance

Qualifications:

• Active, unrestricted Colorado RN license
• 2+ years experience in chronic care management, neurology, or care coordination
• Experience with neurodegenerative diseases (Alzheimer's, dementia, Parkinson's)
• Prior clinical supervision or leadership experience
• Strong documentation, compliance, and communication skills
