Signify Health, part of CVS Health, is seeking a part-time Clinician (NP or PA) to perform in-home health evaluations in the Denver area. Visits take about 45 minutes and may include history review, physical exam and optional tests, with no prescribing required.
You will visit patients at home, connect with families, and use an iPad workflow to document findings. Travel locally and occasional virtual visits are expected.
