Position: Medical Social Worker (60814)

Medical Social Worker

Location: Denver Metro, CO 80223

Salary: $70,000 - $75,000 per year (DOE: $58,000-$75,000)

Responsibilities
  • Develop a plan of care in collaboration with the interdisciplinary team to meet the needs of hospice patients and families.
  • Assist hospice team members in understanding the social and emotional factors related to patients' terminal illness.
  • Provide emotional counseling to patients/families and assist in coping, decision making, advanced directives, funeral planning, or facility placement.
  • Conduct assessments of patients' and families' psychosocial, financial, environmental, and community needs.
  • Make appropriate referrals to community resources based on assessment.
  • Comply with applicable legal requirements, company standards, policies, and procedures.
  • Complete all documentation accurately and legibly; comply with hospice policies, laws, regulations, and standards of practice.
  • Conduct bereavement assessments, develop plans of care with the IDT, and document all bereavement activities.
  • Design and distribute materials to families eligible for or receiving bereavement services.
  • Develop educational programs for families and staff on loss, grief, and related topics.
  • Provide consultation and education to the community to increase knowledge related to hospice; serve as information source and education resource person for the community.
Qualifications
  • Bachelor's degree in Social Work or Psychology (mandatory);
    Bachelor's in Social Work is preferred.
  • LSW license in good standing in the State of Colorado.
  • Documented training in grief and loss.
  • Hospice experience required.
  • Social Work License preferred.
