What you will do:

We are looking for a YOUTH INTENSIVE CARE COORDINATOR who can help shape our vision and support our mission. Here is what the position will look like.

Coordinate care for high-acuity youth, including those eligible for QRTP, PRTF, and CO-SOC services.
Develop, implement, and monitor strengths-based care plans based on CANS and the ESA (Enhanced Standardized Assessment).
Navigate complex systems (e.g., Child Welfare, CDYS, I/DD, education) to support integrated care.
Manage in- and out-of-state placements, including funding coordination, transportation, and regulatory forms.
Collaborate with interdisciplinary teams and participate in solution-focused planning meetings.
Liaise with providers, families, and stakeholders to ensure effective communication and service alignment.
Conduct comprehensive clinical assessments and apply knowledge of behavioral health conditions.
Support youth through the full CO-SOC care coordination period and post-discharge follow-up.
Educate members and providers on services, benefits, and appeals processes.
Track CO-SOC eligibility, participation, and ensure compliance with contractual timelines.
Provide clinical consultation to care coordination staff and contribute to team training and development.
Coordinate funding across multiple sources and understand cost structures related to residential treatment services.
Participate in continuous quality improvement efforts and contribute to required reports and deliverables.

What you will bring:

Education:

Master’s degree in social work, human behavioral sciences field or another healthcare field and/or clinical license (e.g., RN, LCSW, LPC) is required. A combination of bachelor’s degree in related field, plus 8 years of progressive work experience in social work, case management or behavioral health related fields can be substituted in lieu of master’s degree.

Experience:

Three years of clinical, care management and/or healthcare experience required. Preferred experience in clinical inpatient, outpatient, or community health settings.

Experience:

working with Medicare and Medicaid populations, and related social/economic issues delivering care management services, conducting assessments, interviewing members, developing service plans, coordinating care and monitoring services for members. Candidates who are familiar with working across multiple systems such as child welfare, juvenile justice, education, and healthcare are preferred. Bilingual fluent in English and Spanish preferred.
