About the Position
In-Home Health Assessments - Nurse Practitioner or Physician Assistant

Location:

Colorado

Schedule:

Monday through Sunday, 7:00 AM to 7:00 PM (per Order)

Daily Volume:

5 to 8 assessments per day (per Order)

Statewide Opportunities

Opportunities are available throughout most counties statewide. Assignments are scheduled based on provider availability and county demand, with opportunities in all but a limited number of restricted counties.

Position Overview

In-home health and wellness assessments for health plan members

Assessments may include history and physical, medication review, vitals, and basic screenings

No prescribing and no ongoing treatment (per Order)

Documentation is completed on an iPad using a proprietary system (per Order)

Availability Requirements

Ideal: available 2 full days per week (or more)

Must have at least one weekday available

Weekend-only availability is not accepted (per Order)

Requirements

Active, unrestricted Colorado Nurse Practitioner or Physician Assistant license

National certification for NP or PA

NPI required

Must be able to work independently and complete same-day documentation

Travel and Lodging

Travel and lodging support may be available based on assignment distance and provider availability (per Order).
