Job Title: Home Health PT Location: Denver, CO (I-25 East Greeley to Castle Rock Split between Denver and Loveland offices) Setting: Home Health Pay Range: Schedule: 08:00:00 to 17:00:00 Contract Length: 13 weeks Candidate Type: Traveler Requirements: 1 year of Medicare Certified Home Health experience. HCHB EMR experience highly preferred. Responsibilities:
- Provide outpatient physical therapy services to home health patients.
- Conduct assessments and develop individualized care plans.
- Document patient progress and adjust treatments as needed.
- Collaborate with healthcare team members to optimize patient outcomes.
