SummaryThe Asian Community Case Coordinator/Interpreter serves as the communication link between non-English-speaking clients, their families, the agency, caregivers, Medicaid Case Managers, and outside service providers. This position provides interpretation, coordinates services and schedules, assists with referral documents, and helps clients and caregivers understand agency and EVV requirements.
Responsibilities- Client & Stakeholder Communication
- Serve as a primary communication liaison between clients, families, caregivers, agency staff, Medicaid Case Managers, and outside providers.
- Assist clients in communicating their personal care and homemaker needs and ensure service changes are communicated appropriately.
- Service Authorization & Documentation Coordination
- Coordinate referrals, PARs, care plans, service authorizations, recertifications, and required documentation.
- Review records for missing, expired, or inconsistent information and follow up to ensure services remain authorized and compliant.
- Scheduling & Service Coordination
- Coordinate client and caregiver schedules based on authorized services and hours.
- Manage caregiver onboarding, orientation, training, and start-of-care scheduling while coordinating with other healthcare and community providers to prevent scheduling conflicts, duplicate services, or unauthorized hours.
- EVV & Regulatory Compliance
- Educate and support caregivers on Electronic Visit Verification (EVV), including clock-in/out requirements, GPS/location requirements, and visit documentation.
- Resolve EVV discrepancies and ensure compliance with Medicaid, IHSS, HIPAA, and agency requirements.
- Documentation, Monitoring & Compliance Reporting
- Maintain accurate documentation of client communications, scheduling changes, service coordination, and compliance concerns.
- Monitor for changes in client condition, suspected fraud, service duplication, or other risks and elevate concerns promptly in accordance with agency policies.
- Fluent in English and at least one language commonly spoken within the agency’s Asian community.
- Able to interpret spoken and written communication accurately.
- High school diploma or equivalent.
- Experience in home care, Medicaid services, scheduling, interpreting, or care coordination preferred.
- Strong communication, organization, documentation, and computer skills.
- Ability to maintain confidentiality and professional boundaries.
- Knowledge of IHSS, EVV, PARs, and home care services preferred.
Pay: $15.50 per hour for approximately 21 hours per week
