Nurse Practitioner – In-Home Health Assessments

Compensation:$80 per completed assessment

Are you a compassionate and experienced Nurse Practitioner looking for flexible opportunities? We are seeking skilled Nurse Practitioners to partner with in performing in-home health assessments in Denver. This per diem role offers independence, competitive pay per assessment, and mileage reimbursement for travel.

In this role, you will:

• Perform thorough health assessments in patients’ homes, including reviewing medical histories, conducting physical exams, and screening for chronic conditions.
• Educate patients on medication management, disease prevention, and overall health maintenance.
• Document relevant diagnoses and communicate findings to ensure effective care management.
• Collaborate with primary care providers to address any gaps in patient care.
• Enjoy a flexible schedule—no on-call, weekends, or holiday shifts required.

Position Requirements:

• Availability to complete at least 30 in-home assessments per month.
• Active Nurse Practitioner license in CO.
• Board Certification from AANP or ANCC in Family, Adult-Gerontology, or related specialties.
• Minimum of 1 year of clinical experience, preferably with geriatric patients.
• Strong communication and interpersonal skills, with a dedication to delivering patient-centered care.
• Reliable transportation for local travel.
• Comfort with working in varied home environments.
