Job Description

We are seeking dedicated and compassionate nurses to join our team in providing exceptional care to seniors across Colorado. As a Home Care Nurse, you will play a crucial role in delivering personalized care plans tailored to meet the unique needs of each client. Your responsibilities will include:

  • Conducting comprehensive assessments of clients' health status and needs.
  • Developing and implementing individualized care plans in collaboration with clients, their families, and other healthcare professionals.
  • Administering medications, treatments, and therapies as prescribed.
  • Monitoring clients' vital signs and health indicators.
  • Providing assistance with activities of daily living, such as bathing, dressing, and grooming.
  • Educating clients and their families on health management and disease prevention.
  • Communicating effectively with clients, families, and interdisciplinary team members.
Qualifications
  • Current RN license in the state of Colorado.
  • Minimum of 3 years of nursing experience, preferably in home care or geriatrics (1 year in Home Care is required).
  • Strong clinical assessment and critical thinking skills.
  • Excellent communication and interpersonal abilities.
  • Ability to work independently and collaboratively as part of a multidisciplinary team.
  • Compassionate and patient-centered approach to care.
  • Reliable transportation and willingness to travel to clients' homes.
  • CPR certification (required).
  • Experience with electronic health records (preferred).
