Job Summary and Qualifications
* The Clinical Case Manager is responsible and accountable for coordination of services through an interdisciplinary process which provides a clinical, financial, and psycho-social approach through the continuum of care. Balances individually identified patient and family needs with the efficacious and cost effective utilization of resources. Through concurrent case management, patients will be assessed to determine appropriateness of admission, continued hospitalization, clinical resource consumption, progressive care management, as well as appropriate level of care.
Discharge planning will begin at the time of (or prior to) admission, and reassessed ongoing throughout the course of hospitalization. Quality and Risk Management issues and education needs will also be monitored for and reported to the QRM department as a part of the CQI process.. The Case Manager performs delegated duties consistent with the scope of practice and functions within the philosophy and overall plan of care at HCA Health

II.
Position Requirements :
* Licensure/Certification/Registration: RN License in the State of Colorado. Compact license acceptable but must obtain Colorado state license within 90 days of hire.
* Education:
Graduate of an accredited School of Nursing.
* Experience:
3-5 years recent clinical or case management experience.
* Knowledge, Skills and Abilities:
Must have the ability to assess information, apply critical thinking processes to determine course of action, and relay this information to the interdisciplinary team. Requires excellent human relations and oral/written communication skills. Must be able to prioritize workload, and develop efficient methods of managing time.
Must have the ability to compile, evaluate and report statistics to members of the team, and utilize the information to facilitate process improvement activities. Strong communication skills are necessary for meeting facilitation, educational presentations, team collaboration and patient interviews. Must also have strong analytical and organizational skills.
