Job Title: RN Registered Nurse Patient Care Manager – Home Health
Employment Type: Direct Hire
Location: Denver, CO
Salary: $110,000 – $115k
Position OverviewWheeler Staffing Partners is seeking a dedicated and experienced RN Patient Care Manager to oversee home health clinical operations in Denver, CO. This role is responsible for ensuring high-quality patient care, leading clinical teams, and maintaining compliance with regulatory standards. The ideal candidate is a strong clinical leader with home health experience who thrives in a fast-paced environment and is passionate about delivering patient-centered care.
Key Responsibilities Clinical Oversight & Patient Care Management- Review and approve physician orders, including denied orders and follow-up actions
- Review and approve evaluations (SOC, ROC, Recertifications) and supporting documentation
- Manage and monitor patient care episodes, including therapy, nursing, and aide utilization
- Review and approve Plan of Care (POC) edits and clinical documentation
- Conduct utilization management reviews to ensure appropriate care delivery
- Make recertification and discharge decisions based on patient progress and outcomes
- Monitor wound scores and address clinical variances
- Supervise and support RN and therapist case managers
- Provide coaching, mentorship, and performance feedback to field staff and new hires
- Ensure proper onboarding, training, and ongoing staff education
- Approve PTO requests with oversight of case management needs
- Review clinical documentation for new employees and ensure compliance
- Facilitate and follow up on case conferences and care coordination activities
- Manage scheduling updates, calendar changes, and staffing needs
- Review and revise aide care plans as needed
- Maintain accurate documentation, including review, edit, and lock processes
- Participate in QAPI (Quality Assurance and Performance Improvement) initiatives
- Oversee coordination notes, admissions follow-ups, and pending patient activity
- Active Registered Nurse (RN) license in the state of
- Minimum 1 year of home health experience (required)
- Minimum 1 year of supervisory, management, or lead experience (required)
- Strong knowledge of home health clinical operations and regulatory compliance
- Proven ability to lead, coach, and develop clinical teams
- Excellent organizational, communication, and problem-solving skills
- Ability to manage multiple priorities in a fast-paced environment
- Bachelor’s degree in Nursing, Healthcare Administration, or a related field (preferred)
