Job Overview

Signify Health, part of CVS Health®, is hiring a Full‑time Clinician (Nurse Practitioner or Physician Assistant) to conduct in‑home health evaluations. Clinicians will travel to clients’ residences to perform a 45‑minute assessment that includes a medication and medical history review, physical evaluation, and optional diagnostic tests.

• Work with Signify Health as a clinician colleague, fulfilling the requirements of the specified role.
• Be flexible to travel locally and within licensed states, as needed.
• Conduct virtual visits as needed.
• Provide all Signify Health services as indicated, including diagnostic and preventive services products (e.g., spirometry, DEE, etc.).

• Active, unrestricted license(s) in the coverage area(s).
• Board certification (required).
• Multi‑state licenses (preferred).
• Able to cover the following primary location:
  Greeley and surrounding area.

Anticipated weekly hours: 40. Full‑time position.

The typical pay range for this role is $87,035.00 – $. The actual base salary offer will depend on experience, education, geography, and other relevant factors.
