This is a full‑time Registered Nurse role for our Colorado team. Monday through Friday 8AM to 5PM, weekends off. The role includes 401k, health, dental, vision and life insurance. It is eligible for a sign‑on bonus of $5,000 after successfully completing 6 months with Freedom Care.

• Under the direction of the Lead Registered Nurse, support home care consumers in development of Care Plans for start of care and continued authorizations.
• Outreach to consumers to ensure development of Care Plan in a timely manner.
• Engagement and positive relationships with Case Managers and Providers to ensure continuity of care for home care members.
• Conduct home visits for consumers in designated region to support supervisory visits and ensuring compliance.
• Document all interactions consistent with policies and procedures and ensuring HIPAA regulations are adhered to.
• Escalate critical incidents by reporting to appropriate contact.
• Cross‑collaboration with pre‑care, care and clinical team to ensure appropriate care is provided.

• Valid and active RN License in the state of Colorado.
• 2+ years clinical experience;
  Home Care experience preferred.
• Proficient in use of Microsoft Office.

• Ability to provide support to our patients in Denver and Northern Colorado; onsite, telephonic and/or virtual.
• Experience working in startups.

The base pay range for this role is between $80,000 and $90,000 per year at the commencement of your first year of employment. In addition, the role includes 401k, health, dental, vision and life insurance.
