Position Overview
Licensed Practical Nurse (LPN) for private duty home health nursing. Work with pediatric and adult patients with complex medical needs in their homes.
Responsibilities- Provide skilled nursing care under the supervision of an RN.
- Administer medications and treatments based on individualized care plans.
- Educate and support families on in‑home care techniques.
- Observe and report changes in patient condition.
- Document visits using the EMR system.
- Maintain accurate patient records and participate in medical record audit.
- Attend staff meetings, patient care conferences and in‑services.
- Perform preventive and rehabilitative procedures as appropriate.
- Assist in preparing equipment and materials for treatments, observing aseptic technique.
- Participate in patient care conferences involving other health team members.
- Take on‑call duty, nights, weekends and holidays as assigned.
- Execute orders and ensure patient/family involvement in care plan.
- Report incidents or variances to Nursing Supervisor/RN.
- Active LPN license in the state where practicing.
- Active CPR certification with practicum.
- One year nursing experience preferred.
- IV certification required by state mandate, if applicable.
- Excellent communication and critical thinking skills.
- Passion for working with pediatric and/or adult patients in a home setting.
- Physical abilities to perform described duties.
- Acceptance of company vision and goals.
- Ability to exercise independent judgment and initiative.
- Average computer skills; competency in media communications.
- Excellent verbal and written communication.
- Ability to exercise initiative and independent judgment.
- Graduate of an approved school of practical nursing.
- Currently licensed in the state(s) where practicing.
- One year experience in nursing (preferred).
- Active and valid CPR certification with practicum.
- IV certification by state mandate, if required.
