Seeking an experienced Nurse Practitioner or Physician Assistant to join a Heart Failure Program in a cardiology clinic and hospital setting. The role involves managing stage 3 and 4 heart failure patients through outpatient follow-ups, inpatient rounding, medication management, and lifestyle monitoring. It emphasizes continuity of care from hospital to clinic and back to primary or cardiology providers. The position offers a mix of outpatient consults and inpatient duties, with no call or weekend requirements.
Qualifications & SkillsGraduate from an accredited NP or PA program, licensed or eligible for licensure in CO, DEA and prescriptive authority, BLS and ACLS certification, with a minimum of 2 years cardiology experience, preferably in a Heart Failure setting.
ResponsibilitiesCollaborate with cardiologists, ensure medication adherence, monitor patient progress, and support patient education and lifestyle changes. Participate in inpatient and outpatient care, contributing to a comprehensive Heart Failure Program.
